ORIGINAL DOCUMENTS FOR PRESENTATION/SUBMISSION UPON ENROLLMENT

1. PSA Birth Certificate (original and photocopy)
2. Colored 1x1 ID picture w/ white background (2 pcs)
3. Report Card from the school last attended
4. Signed Data Privacy Agreement
5. Student's Medical Background and Certification of Good Health issued by family doctor or pediatrician

Note:  Items 1-3 must be available for uploading in the Online Admissions Form.  When presenting these original documents, provide one (1) photocopy of each original document.

ORIGINAL DOCUMENTS FOR SUBMISSION BY THE FIRST QUARTER OF THE SCHOOL YEAR:

1. Certificate of Good Moral Character from previous school*
2. Certificate of Completion (original and photocopy)**
3. Passport (original and photocopy)***
4. Alien Certificate of Registration (original and photocopy)***
5. Study Permit issued by the Department of Foreign Affairs (DFA)***
6. Authenticated copy of Grades by DFA***
7. Certificate of Evaluation of Grades and Level Placement issued by the Department of Education***
